- Lepromatous leprosy is a more systemic and widespread disease as compared to the tuberculoid form and is seen in patients with poor cell-mediated immunity to M. leprae. The antibody titers to M. leprae are high. The skin lesions are multiple, widespread, symmetrical, and firm and include a combination of macules, papules, plaques, and nodules

- Borderline lepromatous leprosy manifests with a large number of skin lesions consisting of plaques and ill-defined infiltrated nodules. Unlike lepromatous leprosy, the lesions are not symmetrical. Nerve trunks are palpably thickened

Histologically, the tuberculoid and lepromatous forms have distinct appearances. Lepromatous leprosy is rich with dermal parasitized macrophages, whereas the tuberculoid form is similar to the granulomatous inflammation by which its name is ascribed; epithelioid histiocytes with multinucleated giant cells and a lymphohistiocytic cuff Leishmania is an organism related to the trypanosomes. The inflammatory disease caused by the organism may involve skin and mucous membranes, as well as internal organs Frequent atrophy of adnexal structures, increased fibroblasts, thickening and luminal narrowing of small vessels, blunting of dermal-subcutis interface, diminished elastic tissue. Often atrophy of epidermis, mucin deposition. Variable dystrophic calcification of blood vessels. Control biopsy from adjacent normal skin is helpful
